ABSTRACT

This study describes the perceptions of public health services managers and professionals concerning provision of voluntary surgical sterilization in the Campinas Metropolitan Area, São Paulo State, Brazil. The study adopted a qualitative approach in four municipalities (counties), where semi-structured interviews were conducted with 26 health professionals and health services managers involved in the provision of surgical sterilization. The interviewees identified difficulties in scheduling visits at Outpatient Family Clinics or Reference Centers (APF/CR), and the number of available surgeries in the accredited hospitals was insufficient. They emphasized the lack of physical infrastructure and human resources for conducting family planning activities in the primary health units as well as in the APF/CR.They also criticized the legal criteria for authorizing surgical sterilization, and mentioned adaptations to make them more appropriate to the each municipality's situation. According to the health services managers and professionals, despite the efforts, meeting the demand for surgical sterilization in the Campinas Metropolitan Area was jeopardized by its centralization in the APF/CR, which in practice had to cover the gap in family planning activities in each municipality's primary care units.

ABSTRACT

INTRODUCTION: The present study aims to prospectively compare the sexual function in males before and after vasectomy surgery using the international index of erectile function (IIEF). MATERIALS AND METHODS: From October to December 2002, sixty-four patients who were candidates for male sterilization in the vasectomy program of the Urology Section at the General Hospital of the University of São Paulo were included. The same investigator applied the IIEF before and 90 days after the surgery. The mean scores obtained on pre and postoperative visits for all domains of sexual function were analyzed and compared with the Wilcoxon test. RESULTS: The mean patient age was 35 years (range from 25 to 48 years) and the mean number of children per man was 3. The total mean score of the IIEF was 64.06 before surgery and 65.64 after the procedure, with this difference considered statistically significant (p < 0.001). Sixty-seven per cent of the patients improved their scores, versus 17 percent and 16 percent who showed worsening or no change at all in IIEF scores following surgery, respectively. Of the 5 sexual function domains, desire and sexual satisfaction presented statistically significant improvement. CONCLUSION: This study showed that vasectomy caused a positive impact on sexual function, especially on desire and sexual satisfaction, in the majority of men undergoing surgery. There was no case of surgery-related erectile dysfunction.

ABSTRACT

Voluntary surgical contraception (VSC) is said to be one of the most popular, safe, effective and long term methods of birth control not only in Nepal but also in the world One of the very frequently suspected complication or effect of voluntary sterilization (female) is "post tubal sterilization syndrome". The term post tubal sterilization syndrome has been used variously to include abnormal menstrual bleeding, dysmenorrahoea, premenstrual distress, hysterectomy and miscellaneous other conditions like need for recanalisation, feeling of regret and menopausal syndrome. Abnormal vaginal bleeding due to ovarian dysfunction has been a regular and common complaint of the female clients after their TS. This study was carried out in a clinic setting to find out the level of satisfaction among the cases of VS. Level of satisfaction was measured by whether the client regretted the procedure, whether their menstrual pattern was changed after the procedure, performed either by themselves or VSC by the husband and also by finding out how many of them needed hysterectomy and recanalisation procedure. Women who had undergone TS were taken as the cases (group A) and those women whose husband has VSC were taken as control group (group B). The result showed that there is almost no difference in the age group of both case and control at the time of their consultation. At the time of VS almost 60% (56.53%) of cases were <29 years of age and >70% (71.23%) of the women were <29 years of age when their husbands were vasectomised. 26.15% of the women in group A were in the age group of 30-40 whereas this age group comprised of only 15.38% in group B. Average age at menarche was 14 years and average age at marriage 17 years. The menstrual pattern was not found to have changed in both groups excepts for those who had organic condition. Hysterectomy had been performed in only 5 cases at the age of 30, 34, 40, 42 and 43 years in group-A and in two case in group-B. Indications of hysterectomy n these cases were (DUB) and fibroids. Two patients in group-A had to undergo recanalisation but the indication in both the cases were loss of previous children.
